LAW 125(1) OF 2018 OF THE CYPRUS REPUBLIC AND REGULATION (EU) 2016/679 OF THE EUROPEAN PARLIAMENT OD 2016 FOR THE PROTECTION OF NATURAL PERSONS WITH REGARD TO THE PROCESSING OF PERSONAL DATA AND FOR THE FREE MOVEMENT OF SUCH DATA

Express Consent and Declaration of Acknowledgment of Notification in relation to the Use and Processing of Personal Data.

I have been informed of all the provisions of the aforementioned Regulation including my rights and I herewith provide Royal Insurance Company Ltd (and any successors and assignees thereof) and the Insurance Intermediary to which I have submitted my Insurance Proposal Form, my express and unconditional consent regarding the use, processing and creation of data records in electronic and physical form, of all my relevant personal data, that may include sensitive data which I have declared or may declare in the future, on my Insurance Proposal Form to Royal Crown Insurance Company Ltd, whether directly or through an alternative source, as well as regarding any personal data may be declared in the duration of my collaboration with Royal Crown Insurance.
Further, I hereby confirm that I am authorised to forward, at this point, and in the future, any personal data of other persons, which data is necessary for the provision by Royal Crown Insurance Company Ltd. of services to myself.

My consent includes –

• Any legal connection between records collected by Royal Crown Insurance Company Ltd and records kept or may be kept in the future by other legal or physical entities that Royal Crown Insurance Company Ltd is or may in the future be in direct collaboration with for the purposes of execution of the insurance contract and ancillary actions.
• Access to and use of my data by the employees and authorised by Royal Crown Insurance Company Ltd, persons, for the purpose of provision of service to myself and for the purpose of the execution of the insurance contract of which I am the contracting party and/or for any action that may be deemed necessary prior to the inception of the contract.

The above records shall be held by Royal Crown Insurance Company Ltd for the purpose of matters relating to my insurance contract and any other transaction with the company, as well as receipt by me of any information regarding my insurance contract or coverage.
